Billing and Insurance Information
For our patients' convenience we participate with Medicare and most managed care plans. Please always bring your insurance card with you for all your doctors' appointments. If required by your insurance, a referral from your primary care doctor must be submitted at the time of your visit. The co-payment required by your insurance will be collected on the day of your appointment.
Payment Options
We accept cash, check and credit care payments for services rendered. Payment in full is expected at the time of your visit unless other payment arrangements are agreed upon prior toyour appointment.
